Any student organization wishing to be recognized by SGA must fill this out and return it to the Campus Life Office with the other required materials found on the Student Organization Form. Read the Hazing Policy. Then print and sign the last page (Hazing Agreement Form) and submit it with the rest of the Organization Recognition materials.
To become a recognized organization for the 2011-2012 school year all forms must be completed and returned to the SGA Office in Campus Life by 5:00 p.m. Friday, September 2, 2011.
